CHIEF COMPLAINTS   12 yr old female patient came to the opd with chief complaints of 1)Fever( low grade) 5days back. 2)Cough associated with sputum since 3days. CHIEF COMPLAINTS High grade fever since 2 days  Generalized weakness and body pains (intermittent) since 2 days. HISTORY OF PRESENTING ILLNESS  Patient was apparently asymptomatic 6 years ago then he developed neck pain and giddiness for which he went to RMP and was diagnosed as having hypertension started using T. Telma 20mg .Since then he was alright for 2 years and then he developed chest pain which is dragging type of pain not associated with shortness of breath and palpitation

CHIEF COMPLAINTS A 61yr old male came with chief complaints of brief loss of consciousness 3 days back. His GRBS on admission was 39mg/dl. After connecting on 25D he regained his consciousness. H/o of vomiting 1 episode  HISTORY OF PRESENTING ILLNESS Patient was apparently asymptomatic 3 yrs back Then he developed weakness for which he went to hospital & diagnosed with HTN & was started on Tab. TELMA- AM but was on irregular medication. Then he was fine till yesterday night
